Information submitted through the EAI website

The Evidence Adoption Institute collects the information entered into its expression-of-interest form so that it can review and respond to the inquiry. This may include a name, work email address, organization, role, location and information about an institutional decision or requested form of support.

Please do not submit patient-level information, personal information about affected people, confidential organizational records, proprietary algorithms, commercially sensitive information or documents through the website form.

Submitting the form does not create a client, research, advisory or service relationship. If an inquiry proceeds, EAI may request additional information through a separate and appropriately managed process.

EAI does not use the expression-of-interest form to subscribe people to marketing communications. Information will be used for inquiry review, response, record keeping and appropriate follow-up.
